A Comparative Review of the All-on-4 Concept for Dental Implant Rehabilitation: Long-Term Clinical Outcomes and Digital Workflow Advances

Abstract
The All-on-4 concept is a well-established implant-supported rehabilitation protocol for completely tendentious jaws that uses four strategically positioned implants, including two posterior tilted implants, to support an immediately loaded full-arch prosthesis. This paper rewrites and synthesizes the findings of the two uploaded studies, integrating long-term mandibular outcomes with evidence comparing digital computer-aided surgery and conventional workflows. Across the reported cohorts, implant survival remained exceptionally high, with long-term survival approaching 98% and digital workflows achieving excellent short- to medium-term outcomes. Digital planning, guided surgery, CAD/CAM improvisational, and flap-less procedures were associated with reduced marginal bone loss, lower patient-reported pain, and greater satisfaction, while traditional protocols also demonstrated predictable clinical success.
